ALL incoming 7th grade students must have their TDaP (tetanus, diptheria, pertussis) booster shot or APPROVED Certificate of Exemption on file with the Oklahoma State Department of Health and the Middle School office before students will be allowed to start school on August 14th. All exemption certificates must be renewed with the health department before entering 7th grade. No previous Certificate of Exemption forms can be accepted.
Due to the high volume of students needing immunizations before school, please contact your family doctor, local clinic, or the health department as soon as possible to make an appointment to get the TDaP booster shot.

Oklahoma law and Vian school policy states that "For school enrollment, a parent or guardian shall provide one of the following: 1. Current, up-to-date immunization records; or 2. A completed and signed exemption form." Such evidence is required before the child is allowed to enter or attend Vian Public School.
