The following documents are required to enroll a student. Some of these items may be provided upon receipt of official school records from your student's previous school.

Each school site may require other documents as well that are not included in this list. 

 

Student’s state issued birth certificate

Student’s Social Security Card

Student’s current immunization record

Student’s Certificate of Degree of Indian Blood (CDIB) Card (if applicable)

Parent/Guardian state issued identification (e.g., driver’s license)

Proof of residency (e.g., current utility bill in your name)*

Custody paperwork (if applicable)

Contact information for previous school (if applicable)

 

*If you do not reside in the Vian district, please go to https://oklahoma.gov/education/services/school-choice/student-transfers.html and complete the Student Transfer Application Form.

 

NOTICE: During the summer months, all offices have limited staffing and/or shortened hours and will not return to full-time work until August 1st.
